Notice of Potential Unsafe Condition
Rockwell Automation issues a Product Safety Advisory when it identifies a product(s) anomaly that may potentially cause harm to
personnel, capital equipment loss or environmental damage. Immediate action is required as identified below.
Stratix 5410 Ethernet Switch Internal Electrical Short
Reference: 2016-02-001

This Product Safety Advisory informs you of a potential anomaly that exists with the Stratix 5410 Ethernet Switch. On
a small quantity of product, wiring internal to the switch may short to the metal case of the switch. If a short like this
occurs, the metal case of the switch may rise to the same voltage as the power input terminals on the front of the
switch. This may cause the product to stop operating and / or result in an electrical and / or fire safety hazard.

Product Identification
The potentially affected products are Stratix 5410 Ethernet Switches, catalog numbers 1783-IMS28NAC,
1783-IMS28NDC, 1783-IMS28RAC, and 1783-IMS28RDC. Potentially affected product will show a manufacturing
date ranging from July 1, 2015 through January 18, 2016, and may be identified by the product serial number, CIP
number, or MAC ID. A full list of potentially affected units is included as Appendix B of this document.
Important Product that has been factory upgraded to correct for this anomaly will be identified with a deviation
label near the carton label and product nameplate. If your product contains a deviation label, it is not affected by this
anomaly and no action is required on your part.
For new and unopened product, the product
identification information may be found on the product
carton label.
The catalog number will be in the “CAT. NO.” field on
the left above the UPC bar code. The serial number will
be in the “SN” field in the top right corner under the
serial number bar code. The manufacturing date will be
on the right side in the “DATE” field and will be of the
format YYYY/MM/DD, where YYYY is the year, MM is
the month code, and DD is the day of the month.
For opened and installed product, the product
identification information may be found on the product
nameplate, located on the top side of the product.
The catalog number will be in the “CAT. NO.” field on
the left side of the nameplate. The serial number will be
in the “SN” field, located in the bottom left corner under
the serial number bar code. The manufacturing date
will be in the bottom right corner and will be of the
format YYYY/MM/DD, where YYYY is the year, MM is
the month code, and DD is the day of the month.

Product matching the serial number information in
Appendix B that has been factory updated to correct for
the anomaly will be marked with a deviation label. The
deviation label will be located on the product under the
product nameplate and on the carton near the carton
label. The deviation label will be “D517262”.
Product with this deviation label is not affected and no
action is required on your part.
Product identification may be made through a network connection by using the web browser based Device Manager
tool, RSLinx, or the product’s command-line interface (CLI). This tools may be used to report the product CIP
number or MAC ID number. You will still need to check for the physical deviation label prior to returning product, as
described above.
To use Device Manager:
1. Connect your computer to the same network as the Stratix 5410 switch and open your web browser.
2. In the browser’s address window, enter the IP address of the switch you wish to connect to.
3. When prompted, enter the switch name and password.

You may also use RSLinx to identify potentially affected product. To use RSLinx:
1. Connect your computer to the same network as the Stratix 5410 switch and open RSLinx.
2. Open the RSWho network tool.
3. On the listing of network devices, find the Stratix 5410 switch you wish to identify. Double-click the device
entry to open the properties dialog.
In Device Manager, the product identification
information will be shown in the Switch Information
listing. The catalog number will be in the “Product ID”
field, the CIP number will be in the CIP Serial Number
field, and the MAC ID will be in the MAC Address field.
In the example to the left, the catalog number is 1783-
IMS28NAC, the CIP number is 60159219 and the MAC
Address is F4:54:33:17:D8:80. Note that this product is
not included in the Appendix B listing and is not part of
the potentially affected population.

In RSLinx, the product identification information will be
displayed in the properties dialog for the selected
device. The catalog number will be in the “Device
Name” field and the CIP number will be in the “Serial
Number” field.
In the example to the left, the catalog number is 1783-
IMS28NAC and the CIP number is 60159219. Note that
this product is not included in the Appendix B listing and
is not part of the potentially affected population.
Alternatively, you may use the switch’s command-line interface (CLI) to obtain the product identification information.
Information on the use of the command-line interface may be found at www.cisco.com by searching for “IE3000 show
version”.
In the command-line interface, the product identification
information is displayed in response to a “show version”
command. The catalog number will be shown in the
“Model number” field, the CIP number in the “CIP Serial
Number” field, and the MAC ID in the “Base Ethernet
MAC Address” field.
In the example to the left, the catalog number is 1783-
IMS28NAC, the CIP number is 60159219 and the MAC
Address is F4:54:33:17:D8:80. Note that this product is
not included in the Appendix B listing and is not part of
the potentially affected population.
Description
A potential anomaly exists with the Stratix 5410 Ethernet Switch. On a small quantity of product, wiring internal to the
switch may short to the metal case of the switch. If a short like this occurs, the metal case of the switch may rise to
the same voltage the power input terminals on the front of the switch. This may cause the product to stop operating
and / or result in an electrical and / or fire safety hazard.
Temporary Workarounds
No workaround has been identified. Product must be replaced.
Correction
Potentially affected product as described in the Product Identification section of this document should be removed
from service by disconnecting power until replacement product has been provided. Customers must contact their
local Rockwell Automation Distributor or Rockwell Automation Sales office to make arrangements for replacement
product.

If you have purchased a second power supply, remove the second supply prior to returning potentially
affected product. Replacement product is shipped as new product and will contain only one power supply.
The second power supply may be reinstalled in the replacement product.
Retain the SD memory card from the product. You may use this memory card to transfer the current switch
configuration and operating system to the replacement product.
New product should be returned in its original product carton with the Factory Seal intact.
Make sure to reference this Product Safety Advisory PSA 2016-02-001 when requesting replacement
product.
Refer to the Stratix Managed Switches User Manual, Publication No. 1783-UM007, for information on product
replacement. This manual is available from the Rockwell Automation Literature Library.
Requested Customer Action
Rockwell Automation requests you take the following actions:
Check if you have a product affected by this Product Safety Advisory. Refer to the Product Identification and
Description sections of this document for product identification assistance.
If you have potentially affected product, remove the product from service until replacement product is
received.
If applicable, contact your local Rockwell Automation Distributor or Sales Office for replacement. Make sure
to reference this Product Safety Advisory PSA 2016-02-001 when requesting replacement product.
All returns should be over packed to prevent shipping damage during transit. Make sure you have removed
additionally purchased power supplies and the SD memory card prior to return.
Continue to check incoming shipments for potentially affected product. Product in transit or in non-Rockwell
Automation inventory may continue to contain potentially affected product for a period of time after the
publication of this document.
