2. Submit the MAX Trade Up Claim Form, Certificate of Destruction, a copy of an invoice showing the

purchase of qualifying MAX product to:

• This is a limited time offer. ClearOne reserves the right to discontinue this promotion at any time.

• This rebate is not valid on leased, returned, refurbished or used products, or products purchased through

Internet auctions or any other unauthorized channels.

• Invoice(s) showing the purchase of an eligible MAX conference phone and Certificate of Destruction must

accompany the Claim Form.

• All Polycom SoundStation™ conference phones are eligible for this rebate. Polycom SoundStation™

accessories, such as the extended microphones, and the VoiceStation® are not included.

• Rebate valid on eligible MAX purchases made after August 15, 2007.

• Invoice showing the purchase of an eligible MAX conference phone and Certificate of Destruction must

accompany the Claim Form.

• Each Polycom unit is good for one $50 rebate toward any one MAX phone, limit one trade in per

MAX phone.

ClearOne MAX® TRADE UP CERTIFICATION OF DESTRUCTION

In order to show adequate proof of destruction of equipment, you must submit photo(s) of the destroyed product

(crushed, hammered, etc.) clearly showing the product’s serial number.

I, the undersigned, hereby certify that I have taken the product listed below out of service and destroyed it. I understand

that only by destroying the equipment am I qualified to receive discounts towards the purchase of eligible products. I

understand that I must either send the product back to ClearOne as proof of destruction OR include a photo of the

destroyed product with product’s serial number clearly visible.
